Registration Information

Registration for the 2008 Fall City Meeting is now open. CLICK HERE to download the printable registration form.

Registration Options
Register for the Fall City Meeting via fax or mail, simply complete the Meeting Registration Form and return it to NFDA. Please submit one copy of the Meeting Registration Form for EACH individual. Feel free to make as many copies as needed if more than one individual is attending from the same company. NFDA is not responsible for un-received faxes. Mail your Meeting Registration Form with payment to: NFDA, 401 N. Michigan Ave, Chicago, IL 60611-4267, Fax: 312.673.6740.

Deadlines
Although you may regsiter for the meeting at any time, to take advantage of the early-bird registration fee you must register by August 8, 2008. To ensure you are listed in the Fall City Meeting Program, your registration must be received  by August 15, 2008. Otherwise your listing will be provided in a supplement to the Program.

Payment Options
Payment is due upon registration. You may pay by check, MasterCard, Visa or American Express. Checks should be made payable to NFDA. Your registration confirmation serves as your receipt. Although a separate registration form must be submitted for each individual attending, you may combine all registration costs from the same company.

Cancellation Policy
A $95 cancellation fee is deducted for written cancellations received by August 15, 2008. No refunds are issued after August 15, 2008.

Confirmations
NFDA sends a registration confirmation via email to the address listed on your Meeting Registration Form within one week of receipt. This is the only correspondence sent confirming receipt of your registration and payment information. Registrants are asked to read it carefully to ensure all information is correct. Please call NFDA if you do not receive a registration confirmation. NFDA is not responsible for registrations which have been sent but have not been received by our offices.
